Each youth recommended to the Youth Advocate Program will undergo a personal needs assessment and an individual action plan with performance measures. Along with a pre and post assessment tool, an individualized action plan will identify the number and timing of the respective life skills modules required for each youth and will require regular monitoring and updating with progressive steps toward attaining the youth's goals. Complimentary experiential learning exercises will accompany each module to provide the youth an opportunity to practise the new skills. Subsequently, the Youth Advocate will identify opportunities to reinforce the learned behaviours. The skills that youth develop through this “tell-show-tell” method will be transferable to their interactions with family, school and community. Multiple agencies and respective resources will be committed depending on the prescribed need identified in each youth's action plan.
